CPTS Union Santé Nord 77 is offering follow-up consultations for patients with a Long-Term Illness (ALD) who do not have a primary care physician and reside within its service area, including Chauconin-Neufmontiers.

This new system aims to ensure the medical follow-up of patients suffering from an ALD, residing in the CPTS USN 77's operational zone, and who are without a primary care physician. These consultations will notably allow for prescription renewals and blood test monitoring related to their condition.
To make an appointment, patients are invited to call 01 41 81 13 26. It is specified that the doctor consulted under this program cannot become the patient's regular primary care physician.
